PRIVACY NOTICE
New regulations regarding Data Protection came into force in May 2018 which requires companies to inform you about any personal information that we hold about you, why this information is taken, who has access to the information and how it is stored and used securely.
What data will be collected/processed?
Your name, address, telephone number, email address, next of kin details, date of birth, GP details, medical history/details (including current medication). All medical personal and medical data is held
Why do you need this information and how is it used?
To enable ThinkFeet Foot Health to appropriately decide upon or provide any advice regarding your treatment. With your consent, we may think it is necessary to contact your GP with regard to any findings that could need an onward referral if outside of our remit of treatment. This is always completed in consultation with you and with your permission. We will also use your contact details to provide you with appointment reminders as necessary via telephone/SMS.
Who will have access to the data?
ThinkFeet Foot Health has sole access to the data held that is held. Your information will never be passed to a 3rd party or any partner of ThinkFeet .
How will data be stored?
All information (hand-written record cards) are stored in locked cabinets. Patient telephone numbers and addresses are also stored on the mobile device of your practitioner.
How long will data be stored?
Legally we are required to store your medical data for 7 years (or in the case of a minor until that minor reaches the age of 25 years) from the last date of your treatment. Please note your right to be 'forgotten' cannot override the legal requirement to keep medical notes for the mandatory period.
When will the data be destroyed?
All data will be destroyed after 7 years or in the case of a minor when they reach the age of 25 years.
